LVN (Licensed Vocational Nurse) Program Requirements
Basic Requirements
- At least 18 years of age
- Valid government-issued ID
- Valid SSN or ITIN
- High School Diploma or GED
- Current CNA state certification
- Enrollment form and interview
Academic Requirements
- 3-unit Anatomy (min. “C”) AND 3-unit Physiology (min. “C”), or combined A&P ≥4 units (min. “C”)
- Courses within past 5 years
- ATI TEAS exam (meet policy requirements)
- 100% on Basic Math for Nursing test
After Acceptance
- Malpractice insurance (CAE)
- ATI LAUNCH program completion
- Active health insurance
- Background check screening
- Physical exam and screenings
- Current CPR/BLS
CNA (Certified Nursing Assistant) Program Requirements
- At least 16 years of age
- Valid government-issued ID
- Valid SSN or ITIN
- If minor, parent/guardian present during enrollment
HHA (Home Health Aide) Program Requirements
- Valid California CNA license
- High School Diploma/equivalent or pass exam per CCR §94904
Phlebotomy CPT-I Program Requirements
- At least 18 years of age; valid government ID
- High School Diploma, GED, or college transcript
- Health exam and screening
- BLS Certificate (AHA or Red Cross)
IV Therapy & Blood Withdrawal Requirements
- Valid California LVN or RN license
- High School Diploma or equivalent, or pass exam per CCR §94904

Health Requirements Checklist
- Physical exam within 6 months of start date
- Immunizations/Screenings: TB, Influenza, Varicella, MMR, Hep B, Tdap, Covid-19
- BLS for Healthcare Providers – current (AHA)
- Background check & drug screening (as required)
- HIPAA & facility orientations (as assigned)
Live Scan (Background Check)
California Live Scan is required for certain roles and certifications. Bring completed form to authorized Live Scan location.
Additional Requirements
- Maintain active health insurance during program
- Complete facility-specific orientations
- Follow individual hospital/clinic policies
- Submit all clearances before first clinical day
